Academic Dismissal

Appealing your Undergraduate Academic Dismissal

1. Log in to MyFIU and find the Task tile on your dashboard.
2. Under the To Do List select the link labeled "Dismissal Appeal" followed by current semester.
3. A new page will load and display the required documents. Click on the link for the "academic plan and academic agreement."
4. Print and complete these forms. The academic plan MUST be completed by an academic advisor, department Dean or designee.
5. Once BOTH forms have been completed, return to the To Do List and click "Dismissal Appeal" again. Click OK to complete the reinstatement agreement form. It is recommended that you submit the forms along with all supporting documentation online.
   • To add an attachment, click on the attachments button located at the bottom of the form. Click "Add" in the dialog box to attach a copy of your documents in PDF format. Repeat this process to add all forms and supporting documents. The number of documents you attach will be displayed at the bottom of the form.
6. Click "Submit Form" to complete the reinstatement agreement.
7. A confirmation email will be sent to your FIU email account confirming your submission has been received. If you are required to submit additional documentation, you will be notified via email and your To Do List.
